Applicants will need to complete and sign the CIMC COVID-19 application online or on paper. Paper applications can be downloaded from the Mechoopda website Click Here. Completed paper applications can be submitted to CIMC via email to Seabaze Whitmore at or fax to (916) 641-6338. The online application can be found at Click Here.

If a household receives a Safeway gift card, each household must sign and return an additional Safeway Gift Card Agreement when the gift card is received. A pre-addressed, postage-paid envelope will be enclosed.

When requesting a gas card, applicants must provide the name of the person in the household who will be receiving the card.

For Internet assistance, applicants must submit a copy of the current invoice and, if necessary, the time period that needs to be paid.
